I believe the asciidoc manual is a subtle and great one, but often not make enough sense for idiot like me (I even printed the whole manual and try to read cover to cover, hoping to learn systematically...).
working examples do better.
anyway, I think I finally work it around (50% by guess) via:
footnoteref:["visual-select", "this is done by press v,V or ctrl-v from normal mode"]

Now I realized it might be this in the manual as you just pointed out:
["22 times", backcolor="#0e0e0e", options="noborders,wide"]
"If one or more attribute values contains a comma the all string values must be quoted (enclosed in double quotation mark characters)."

        I tested with footnote , however, it works well when in
        different lines.

    I just realized this might be by design (footnote has the same
    result in the section title '.my title' , please ignore.

    by then the new issue I found is:

    footnoteref:[visual-select,this is done by press v,V or ctrl-v
    from normal mode]

    will only renders:

    7. this is done by press v
