In all questions please state the version of asciidoc and which backend and toolchain you are using.
Assuming from your png you are generating PDF, then asciidoc is generating docbook. Docbook has two different entities for examples, one with title <example> , one without <informalexample>. Whichever toolchain you use is rendering these differently. See FAQ #3 for how to modify toolchain formatting.
