FWIW, here are trivial sample files showing the problem:

 http://www.panix.com/~grante/files/asciidoc/pointer.txt
 http://www.panix.com/~grante/files/asciidoc/pointer.pdf
 http://www.panix.com/~grante/files/asciidoc/pointer.html

The pdf and html files were generated with the commands 
  
 a2x --format=pdf --fop pointer.txt 
 asciidoc pointer.txt

It's entirely possible this is due to lack of appropriate fonts on the
machine where fop is being run, but there are no warnings or messages
from either of the above commands...
