Language patch, thanks for doing the todo :) As Tobias said plase make
it a pull request on Github.

A2x change, thanks, but an equivalent change has already been made to
the git version of Asciidoc.

Epub change, not sure, make a pull request and xsltproc specialists
will have to confirm it.
