As you say its arguable if notes should be indented to match the list
item, or if they should always be most visible at the left hand edge.
Personally I don't have a strong preference.

Not sure that the asciidoctor doing different things depending on the
blank line is a good idea, but I guess that depends on the reason for
it (or maybe its unintentional).
