Hi Sean! It's necessary to activate Asciidoctor Diagram when you invoke Asciidoctor. For example:
asciidoctor -r asciidoctor-diagram document.adoc Have you done that? The Asciidoctor Diagram README should probably mention this step explicitly. I don't see it mentioned currently.
