Well, I don't think PDF cares, its an unstructured format so possibly
the problem is docbook which is the format asciidoc produces and which
is then processed by other toolchains to get PDF.  Or possibly its the
toolchains that are limited.

You could try searching the mail archives to see what the previous
discussions said, your search may be better than mine.

BTW To make it work you will have to define more than just the input
that you mentioned, you have to define the output to be generated.

You could also try the asciidoctor tool, it supports a newer version
of docbook and I think another level.
