That's what I eventually wound up doing, using a very obscure Unicode character:
# Ellipses # Make "..." replace itself with the Unicode character "Notched Left Semicircle # with Three Dots"; we'll leverage that to do the rest of the replacements. (?<!\\)\.\.\.=🕃 # Handle "...." and "!..." 🕃[.]=. . . . [!]🕃=! . . . # Handle "?...", with slight kern to adjust for width of "?" [?]🕃=<span style="letter-spacing: -0.1em;">?</span> . . . # Anything else becomes normal ellipses again. (?<!\\)🕃=. . . # Escaped ellipses. \\\.\.\.=... It's a shame that there's no way to actually delete a replacement from the list order, but I can see that being an advantage in some circumstances.
