I'm not sure what the rationale was for stripping all the formatting
when building TOCs, but I think it looks better this way. I have a patch
to asciidoc.js if anyone's interested. Or I'm willing to hear you out on
why putting formatting there is a stupid idea and how I should be strung
up for the very idea. :)
