Thanks for your response!

I have found the custom xsl file. I grepped it looking for the word 
"Chapter" and found nothing.

The other thing this custom xsl file does is to 
import /usr/share/xml/docbook/stylesheet/docbook-xsl/fo/docbook.xsl.

I have grepped that file too and am still unable to find "Chapter" anywhere.

Are there any command line switches or equivalent that let me see *all* the 
xsl files being processed?

Or if you have any other hints for chasing this down....

You appear to be using a custom xsl file so it probably depends on
