Clearly these work for you, and far be it for me to pick some nits,
but I will anyway, in the hope of improving them some.
A general comment, things in {abc} are called "attributes"

not "macros", its going to confuse others if you use the wrong
terminology. You do need to fix that, even if you ignore all the
other comments :)
There is a "macros" thing in Asciidoctor, but that section in the
Asciidoctor manual is currently still TBD.
1) Using the extension ".txt" for Asciidoc files doesn't allow editors
to identify the filetype from the extension, so you have to select the
filetype to benefit from the Asciidoc support, maybe it would be
better to use .adoc and .asciidoc.
2) Agree, underlines are ok if you only ever talk ASCII, but also with
=== you can rearrange documents much more easily by simply adding or
removing = from the front of the header markup in one block. If your
editor supports regexes replace all ^= by == to go up a level and with
nothing to go down a level as one operation in a whole selection or
the whole document. You can't do that with underlines since you have
to change the character and get the length right.
3) Also my editor collects // TODO lines in a list for my easy planning
4) Agree
5) Your choice.
6) Agree if you use the URL more than once.
7) But don't add titles that just say the same as the note itself,
thats bad form.
8) Agree, but note the comment about .txt extensions in 1)
9) Underscores or dashes is just a preference, YMMV
10) Agree
