I tried with the ultra suggest mode, but with that turned on I do not get
any suggestions anymore. Even for words that need only one character added
or removed to correct them. Am I missing something?

Try setting the "sug-mode" option to "ultra" (or maybe "fast").

Also, the difference in performance between the C++ API and the pipe mode 
is negligible when looking for suggestions.
