The error is very clear and correct.  You need to have a match or a name here.

<xsl:template>
       <xsl:apply-templates/>
</xsl:template>

should be

<xsl:template match="/">
       <xsl:apply-templates/>
</xsl:template>

Which matches the root and gets you going.
