Standard linkage was documented in 1966, in the IBM System/360 Operating System Control Program Services manual. http://www.mirrorservice.org/sites/www.bitsavers.org/pdf/ibm/360/os/R01-08/C28-6541-1_Control_Program_Services_Apr66.pdf I suspect that it was documented earlier than that, but that is the oldest reference that I could find.
