I noticed in my post that the ampersand lines somehow got partially treated by html. 1. The "C2A" test line with comment "does not pair ampersands; okay" should have 4 ampersands delimited by single quotes. 2. The "C2A" test line with comment "generates error; okay" should have 8 ampersands delimited by single quotes. 3. The "SETA" ampersand test should have 8 ampersands delimited by single quotes. 4. Same goes for the HLASM SYSPRINT snippet.
