The last really stable version for me was 1.5.1.2(1.0.07). As I switched to
1.5.1.2(1.1.03) or 1.5.1.2(1.1.05) (can't blame one of them exactly because I
updated them within a short period) I too get:

So this bug must be introduced between 1.0.07 and 1.1.05. It must be an issue,
because I haven't changed any permissions on the pb folder.
Reverted back to 1.0.07 and the permission denied errors are gone.

OS: Windows 2003, NTFS
User Account : Administrator
Service : SYSTEM
No Group Policies
No new events are generated in the Event Log at the time of the error in ASSP
Only 1 account, Administrator (which is under the Administrators group) The
SYSTEM account is not apart of the Administrators group
I can't login as SYSTEM. But the SYSTEM account has the same privileges to the
assp folder that the Administrator account has, and I can delete, create, etc
with that account.
I really don't think this is a problem with my permissions. All the other files
in the pb folder are fine, only the pbdb.black.db file generates the error. I'm
assuming all the other files are getting the same treatment as the pbdblack.db
file.
