Perhaps utilizing Captcha. Options including, whitelist as usual, or whitelist for X amount of time. Require whitelist requester to specify recipient email address(which would be verified via validate recipient methods). Option to send notification to recipient with any instructions, such as "sen...@notspammer.com has been whitelisted automatically and will remain whitelisted for N days. If you wish them to be permanently whitelisted, please forward this notice to whitel...@...".
