If you're Windows based I found a great utility for maintaining your spam/nospam directories. It is called killold.exe. I schedule a batch file to run each night and in the batch file, I call killold.exe with different options on each of the directories on my server that grow out of control.
You could leave subjects on and just use this utility to prune the directory.
