Necessary, No. Recommended, Yes.

As-is would be best but is not required as far as I can tell.
The ">" and other forward info could possible skew the Bayesian a bit, 
but it's not a huge issue.

Make sure they are not reporting spam that ASSP has already marked as 
spam (Bayesian test mode for example).

If your users are using outlook I found the best way to forward as an 
attachment was to create a new message and then drag the spam message 
into the new message as an attachment.

Perhaps I'll add a section on the wiki addressing these questions.
