Could be a few things:
1) Regex test thats not written correctly (usually a non terminated 
wildcard in my experience)

2) clamd socket timeouts...I had this problem recently, ASSP insisted 
that clamd was running when it was not and after a few minutes would 
just stop processing mail until the connections to the 
clamd-that-was-not-there timed out.

3) Net-DNS 0.59 could be installed, this is a known issue and would 
definitely cause issues.

Since he talks about cpu usage i would put my money on #1.

Also for a install that large the disk subsystem plays a large part.
I had ASSP just slow to a crawl on a server that was getting around 
80,-100K mails a day (99% spam) one we upgraded the drives and put it on 
a stripe it worked like a dream (we synced the ASSP files to another 
server hourly in case it died).
