Make sure you have these set in your clamd.conf MailFollowURLs no PhishingScanURLs no They can cause serious performance issues if enabled on some servers. Also make sure you're running the latest version of ClamAV, the performance improvements are quite nice, "clamd -V" will tell you the current version.
