If you don't do virus scanning you will get infected emails through the system, any system. Not all viral emails are spam. Sometimes people send legitimate mail that has an infected attachment. This particular one isn't the case, but if you aren't doing virus scanning then you're going to get things that don't show up as spam, at least until later in the cycle when they start getting flagged because of various blacklists, etc., as you've indicated is the case for this one as well since later instances of it got flagged and blocked.
