If you buy a DID from my network i will forward it in SIP or IAX
format whatever you want it, right to your gateways if you want. You
are all time in the middle of the picture.

I can't speak for Rehan's network, but it is common for the media stream (RTP) to be set up directly between the two end points, even if the SIP traffic is directed through intermediate nodes for signalling/billing purposes. This is under the control of the servers 'in the middle'.
