VoicePulse Connect (http://connect.voicepulse.com) offers unlimited incoming minutes for $11/mo per number. They do SIP and IAX termination. US numbers only I believe. We've been using them for a few months and have only had minor incoming DTMF problems that were apparently due to the particular area our numbers were in.
