Epygi, TalkSwitch, MKC Networks, PBX-n-SIP (Snom) are all "appliance" type iPBX's that are well tested/documented and seem to work very well. D-Link has a PBX appliance shipping soon as well a a bunch of other manufacturers.

Easy to setup/support,
No moving parts... so compact flash is ideal,
Small,
Expandable (probably not realistic given the requirement to be small?)
All the standard features, ie Automatic Attendant, voicemail, conferencing
etc...
