Bob, Our Enswitch product may be suitable. It's built around Asterisk and SER/OpenSER, and is in production today on systems of over 100000 users. It's aimed at hosted PBX providers and ITSPs (Internet Telephony Service Providers), but is also suitable for large CPE PBX systems such as what you describe. More details are at:
