How do you transfer/move an active call to an external number via a
dialplan using either the app "Dial" or "Transfer" then have Asterisk drop
out of the connection. Basically, how can we have asterisk dial another
external number, transfer the caller, then disconnect and no longer be
required - leaving the transferred and called parties connected.
