I believe I would be remiss if I didn't tell you about Vyatta as your router.  
We use OpenSBC running on Vyatta, which is a router built on a customized 
Debian platform.  It's open source, fast, and very reliable.  OpenSBC handles 
all my NAT traversal to our ITSP, and I have been very happy with it.  I run it 
as a virtual machine on a WAN exposed hypervisor.

With pfSense I had some NAT table issues (may have been resolved since then) 
and had call quality issues with a Cisco ASA.  The combination of OpenSBC 
running on Vyatta has addressed all of our needs.
