I suggest you look at OpenH323 / Opal / Woomera. (openh323.org)
They have a cross-platform h323/ sip / iax stack, used by Gnomemeeting,
OpenGK, Openmcu and others.
Some members of that community (Derek Smithies for one, as i recall)
are/were investigating a windows pabx built on the Opal libraries.
ps it's all c++, which is a better fit with ms's developement tools / libraries.
