finally tracked it down to a zombie channel which wasn't being killed
properly. ooh323_fixup() in chan_h323.c is a stub function which does
nothing. i've since expanded this based on what sip_fixup() does in
chan_sip.c, and the problem went away.
the asterisk channel portions of ooh323c in addons needs lots of work,
before it can be considered fully stable.
