Doublecheck the bootrom and SIP software version your phone are using... I think the ability to read the boot-server with a url or username/pass starter in sip 1.2.2 and later...

I've never used the provisioning provided with the gui but if you try to send a string...  like a protocol, username, password updating should fix this


FYI if your phone doesn't "support" the boot-server option you'll see your phone sending request to your DNS server trying to resolve "ftp://username:p...@10.10.0.11"  if you see that just try to update...

I had this kind of problem here last week, updating my phone fix this...
