Way back in the day, many UNIX systems authenticated users by IP address, then SysAdmins figured out it was a bad idea. (IP addresses can be spoofed.)

None the less, how are you recieving incoming calls? A guest account? host= in sip.conf & iax.conf applies to the client, not direction of call, so it should work fine.
