The new format is:
exten => _1NXXXXXXXXX,1,Set(CALLERID(number)=4025551212|a)
exten => _1NXXXXXXXXX,2,Set(CALLERID(name)=NPI|a)
exten => _1NXXXXXXXXX,3,Dial(IAX2/teliaxout/${EXTEN})

The setcidname isn't really needed under most circumstances, but I
send it anyway for consistency.
