I have not experience this with Voip only. I have experienced this with my cell phone though. I have trouble using Voicestrem to dial any DTMF tones (to a Asterisk box or any other IVR type application).
I can dial into the exact same application using another cell phone service and it works great. I doubt that helps much but hopefully it validates the fact that having an issue like this can be specific to a provider.
