Hi,
I had the same problem when I had an inbound call WITHOUT callerid and I had not any line in my extensions.conf starting with s so Asterisk couldn't make a match.
This was my inbound context:

[inbound_menu]
exten => _X.,1,DigitTimeout(2)
exten => _X.,2,NoOp(DS:${DIALSTATUS})
exten => _X.,3,Answer
exten => _X.,4,NoOp(DS:${DIALSTATUS})
bla...

if a call without callerid arrives, my asterisk cannot match it and complains
You need something like:
exten => s,1,...
