I'm starting to see other postings now in other forums (on Digium and AAH)
about this same issue. It's starting to look possibly like an AMP problem,
but there is nothing in their forums about it.
If I find anything, I'll be sure to post here.

As you can see, the channels are set properly. One thing I did notice is
all of the ;;;;;;;;;; in front of the [ext] sections. Does that seem
correct? I removed them and it didn't change anything. Other files that
you would like to look at?
Also, I'm not sure how to get a trace of the call. Again, trying to get my
head wrapped around all of this. The good thing is once I know how to do
it, I don't need to ask again.

verbosity to a resonable value. E.g: 'set verbose 3'.
Isn't this "in the FAQ"? (what exactly is "The FAQ"?)
