I have several production systems with tdm400p.
Do you have any IRQ conflict? cat /proc/interrupts
Did you try changing the pci slot?
Maybe the fxo module is damaged? did you try changing it to another
place of the
card? I don't know if this can help, anyone?

Try to use another land line and test the echo problem again.
Do you have any DSL service running in that line?
