You're calling the script using EAGI not AGI - This caught me out the day. Changing extensions.conf to use AGI solved my problem :-)
Technical explanation: Something to do with EAGI providing audio on file descriptor 3, it confuses things. Stick with using the AGI app to call your scripts and you should be fine.
