I'm not exactly sure what you are looking for, but I know you can use GotoIf to do conditionals based off criteria in the astdb.

I'm already using realtime for some extensions/users/voicemail. 
 
Is there any way to do the following at point 3?:-
 
Lookup the realtime users db and read the MailBox column for that buddy.
If the mailbox column is empty, play a message saying "Sorry, no one is available."
If the column has data in it, do the following:-
 
exten => _11XXXX,3,VoiceMail(MailBoxID)
