In your first post you weren't using the quotes around the option, this
time you are... I didn't notice that:
exten => s,202,GotoIf($[${holdopt} = 1 ]?4)
Try the same line with the quotes this time.
exten => s,202,GotoIf($["${holdopt}" = "1" ]?4)
