Today, Free Telecom, the second largest ADSL provider (which offers IP, telephony and television over ADSL) has opened an access to a SIP server for their customers. Before that, people like me using Asterisk had to use a FXO card[1], now they can do everything without ever resorting to an analog echo-full non-signalled channel :)
People living in France and using Free Telecom can read [2] (in French) for an example of Asterisk configuration using Free Telecom SIP server.
