Did you forget that FXS interfaces are configured with
FXO signalling
and that FXO interfaces use FXS signalling?

Hi Joseph,
Try looking if the FXO and the FXS modules are placed in the same
order that the other TDM400P card; cause the problem appears to be like
you are using an erroneous signaling to one of the modules.
