I have this problem as well but it's intermittent.  I can hang up and dial back and the problem will be gone.  I don't have much to add except to say that if you figure it out, I'd appreciate hearing your solution.

The issue occurs with gsm as well so it seems to occur out-of-band also.

I'm having problems with choppy audio - I think it's packet dropping.  I don't know if that's the cause of the problem since the choppiness and DTMF issue aren't necessarily correlated.

ok - so I think IPKall uses g.711.

Shouldn't one of the below options work?
auto, rfc, or inband?
