Two ways:

1.  Call forward the first extension in a busy condition to another
extension registered on the phone.  I use trixbox and with that it's a
*90 and then the extension to forward to.  (On the second line, send
busy and don't answer to voicemail.)

2.  You can have multiple calls to the same SIP registration using the
Polycom config file.  I believe the parameter in the extension
registration is something like callsPerLineKey="X" where X is the number
of calls to get to that extension.
