We got a prompt response from Sangoma and gave them SSH access to troubleshoot the issue. Modifying relaxdtmf on its own did not help but if used in combination with rxgain it made all the difference. The system is working pretty good now with these two modifications:

relaxdtmf=yes
rxgain=6.0
