Unfortunately your configuration looks OK to me.

Here's mine, which works 100% with CID (but not dratted hangup detection!). There are some duplications and things - just ignore them. I note that you have sendcalleridafter=2 and I have =1 but I think =2 is just fine. The only thing I can suggest is to play with the RX gain in case things are just too quiet:
