Don't setup a dundi-extens context in your extensions.conf unless you're planning on putting priority 2+ extensions in there, since chan_sip will automatically create the context.
In dundi.conf, I have my mappings as:
e164 => dundi-extens,0,SIP,tcm1/${NUMBER}
internal => dundi-extens,0,SIP,tcm1/${NUMBER}
We use realtime for our sip peers, but it should work the same for static sip.conf peers, just configure regexten on a per peer basis and when they register, the extension will show up in dundi-extens. It took a little fighting with the first time I tried it, but once I figured out the mechanics of it, it pretty much installs with the ease of dropping the configs in. The main thing to keep in mind is that if you have the context set up in extensions.conf, you'll probably run into problems getting it to work.

Also, regexten is not meant to be a global configuration option, it must be configured per peer (or if you have regcontext and nothing for regexten, it'll regexten the peer name instead of what you choose per peer).

Can you post an example configuration? Using regexten and regcontext in sip.conf DOES NOT produce anything for me. If you dont mind me asking as well, what version of Asterisk are you using?
