1 for each location, if the T1 goes down, they still will have a phone
system. Each Asterisk system can trunk to each other via IAX.

If you want to supply dial tone (Modems/Faxes/etc) you'll need a channel
bank.

Either Digim or Sangoma Dual T1 card. Depending on your budget, you may
want to look into their hardware base echo cancellers

No, this is very doable.
