If you turn on the chan_misdn debug messages than you can see what chan_misdn sees as the incoming number. My problem was that my dialplan had for example 881234567 while chan_misdn was seeing 0881234567 so there was no match. A quick change from 881234567 to 0881234567 in my dialplan fixed it.
