Are you running Asterisk as non-root? If so: is the included
configuration file readable to the user running Asterisk?

Please set core debug to a decent value, enable debug logging (e.g debug
or full in the sample logger.conf) and reload. Among the tons of
messages you'll find a message regarding any attempt to open an
#include-d file.
